How has no one taught you how to do this? attaching pictures wont do you any good. This is how you should fill it out.
Example:
7/7/15. *place the date so everyone knows what day it was*
core. *if you played in hardcore or core *
25-9. *your k.d for that map which cant go over 10 deaths or it doesnt count*
Ascend. *the map you played on*
STAY FROSTYoo * the patched in members you played with. You have to play with a patched in memeber or it doesnt count*

No, I didn't say that.
I said if you went somthing such as 25 + /11 I would personally allow it, although I was not sure what would be allowed.
